ABSTRACT
A 28-year-old man was admitted to the first medical center of Chinese People′s Liberation Army General Hospital because of multiple myalgia and intramuscular nodules for more than 2 months. The patient complained of dysphagia, fever and weight loss 2 months ago. Magnetic resonance imaging and biopsy revealed nodular fasciitis. Inflammatory indicators including C-reactive protein, erythrocyte sedimentation rate, platelet count and fibrinogen were slightly elevated. Urine occult blood was positive. Abdominal ultrasound revealed left hydronephrosis. Because nodular fasciitis could not explain the whole situation, a needle biopsy of intramuscular nodules was performed. Pathological examination revealed intramuscular metastatic adenocarcinoma with poor differentiation. Gastric endoscope and positron emission tomography-computed tomography confirmed the diagnosis of advanced gastric adenocarcinoma with extensive metastases of esophagus, lymph nodes, muscles, ureter and bone. Although chemotherapy was given, the patient died of disease progression six months later.

Objective To investigate the efficacy of arthroscopic synovectomy on refractory knee arthritis complicated with popliteal cyst.Methods Patients diagnosed as rheumatoid arthritis (RA) or spondyloarthritis (SPA) with refractory knee arthritis who underwent knee arthroscopic synovectomy in our hospital from 2010 to 2017 were enrolled,including 20 patients (16 RA,4 SpA) with popliteal cyst.Clinical data,RA disease activity score (DAS28),SpA back pain score,etc,were collected to evaluate the efficacy of knee surgery.Results Erythrocyte sedimentation rate (ESR) [58(17,79)mm / 1h vs.19(9,30)mm/1h,P＜ 0.001],C reactive protein (CRP) [3.72(0.92,8.14) mg/L vs.0.85(0.10,3.08) mg/L,P＜0.001],rheumatoid factor [64.6(20.2,193.3) vs.20.5(10.0,58.4),P＜0.001],DAS28 score(4.67±1.25 vs.2.81±1.23,P＜0.001),knee joint discomfort score [5(4,6) vs.2(1,3),P＜0.001] and the volume of knee joint effusion by ultrasound (P＜0.05) in 95 RA patients were significantly decreased compared to those before operation.ESR [27(12,54)mm/1h vs.20 (16,28) mm/1 h,P＜0.001],CRP [3.27(1.06,6.95) mg/L vs.1.41(0.34,3.03)mg/L,P＜0.001],knee discomfort score [2(0,5) vs.1(0,3),P＜0.05],back pain visual analogue score (VAS) [5(4,5) vs.2(1,3),P＜0.001],and the volume of knee joint effusion by ultrasound (P＜0.001) in 58 SpA patients were significantly lower than those before the operation.The rate [16.84％(16/95) vs.6.32％(6/95),P=0.023] and grading (P=0.007) of popliteal cyst in RA were decreased after the operation.No statistically difference was observed in the rate [6.90％ (4/58) vs.5.17％(3/58),P=0.697] of popliteal cyst in patients with SpA,yet with a trend of decrease in 4 patients.Conclusion This study provide evidence that knee arthroscopic synovectomy has a good effect for refractory knee arthritis,which can reduce disease activity,improve joint symptoms and decrease the grading of popliteal cyst.

OBJECTIVE: To observe the effects and mechanism of uranium dust on collagen synthesis in fibroblastic cells.METHODS: i) RAW264. 7 macrophages were divided into solvent-control group and dust-affected group,and then treated with 0 and 120 mg / L( final concentration) uranium dust suspension for 0,2,4,8,16,24 and 32 hours. The supernatants of cells from these two groups were collected,and transforming growth factor beta 1( TGF-β1) levels were measured by enzyme-linked immunosorbent assay( ELISA). The best time phase was defined,and the supernatant of RAW264. 7 macrophages culture were separated as the conditioned medium( CM) for subsequent experiments. ii) Normal mouse lung fibroblasts L929 cells were divided into control group and uranium dust group,and treated with the CM of solvent-control group and dust-affected group respectively. After cultivated for 0,6,12,18,24,30 and 36 hours,the expression of α-smooth muscle actin( α-SMA) in L929 cells was detected by immunocytochemistry. The levels of extrac ellular collagen type Ⅰ( Col Ⅰ),collagen type Ⅲ( Col Ⅲ) and hydroxyproline( HYP) were detected by ELISA.RESULTS: i) The level of TGF-β1 in RAW264. 7 macrophages supernatant of the dust-affected group was higher than that of the solvent-control group after treatment with uranium dust for 4 hours,and showed an increasing trend with increasing time during 4-24 hours( P < 0. 05). The peak value was in 24 hours. The best time phase was 24 hours and used for CM of the two groups in subsequent experiments. ii) Compared with the control group at the same time points, the α-SMA expressions of L929 cells in uranium dust group increased after treatment with CM for 18-36 hours( P < 0. 05),the levels of Col Ⅰ,Col Ⅲ and HYP in uranium dust group increased after treatment with CM for 12-36 hours( P < 0. 05); both the expression of α-SMA and levels of Col Ⅲ and HYP increased with increasing time,showing a time-effect relationship( P <0. 05). CONCLUSION: Uranium dust can induce macrophages to secret TGF-β1 and affect fibroblast,increase α-SMAexpression,and increase the Col Ⅰ,Col Ⅲ and HYP synthesis. These might be the important mechanisms of lung fibrosis.

OBJECTIVE: To investigate the efficacy of Anti-H. pylori rFlaA IgY-sucralfate ethyl cellulose(EC) microcapsules on Helicocacter pylori infection in mice. METHODS: After dealing with sucralfate, the rFlaA IgY microcapsules were prepared with EC by organic solvent evaporation method. The protective effect of sucralfate at different concentrations of pepsin in different pH on the rFlaA IgY activity was also evaluated. Treating the mice with the microcapsules by oral at various dosages (0.8, 1.6, 3.2 and 6.4 mg · g-1 · d -1), the anti- H. pylori serum of the mice and the urease of the gastric mucosa were detected to evaluate the clearance rate of H. pylori by ELISA and rapid urease test. RESULTS: The results showed that under simulated gastric juice with common pepsin concentration (0.02 mg · mL-1), the relative activity of the IgY treated with 40% sucralfate can maintain 82.14% for 8 h at pH 3.0 and 63.28% at pH 2.0, it can only keep 21.52% at pH 1.0. While under the simulated gastric juice with high pepsin concentration (0.04 mg · mL-1), the relative activity of the IgY can maintain 77.87% with 40% sucralfate at pH 3.0, 57.76% at pH 2.0 and 22.79% at pH 1.0. The entrapment efficiency of the EC microcapsules was 83.79%. The release rate of the EC microcapsules was 95.92% in 5 h. After treatment with rFlaA IgY EC microcapsules, the levels of anti-H. pylori antibody were significantly decreased in each group. The clearance rate of H. pylori at dosage of 3.2 mg · g-1 · d-1 was 80%, the rFlaA IgY at dosage of 6.4 mg · g-1 · d-1 can completely clear the H. pylori in mice gastric mucosa. CONCLUSION: The 40% sucralfate can protect IgY in the simulated gastric juice. The rFlaA IgY EC microcapsules have high entrapment efficiency and sustained-release effect in vitro as well as good clearance effect of H. pylori infection by oral administration.

Objective:To investigate the ameliorated effects of n-butanol extract from Radix Pseudostellariae(RP) on rat experimental cardiopulmonary Injury induced by acute myocardial infarction.Methods:The experimental rat cardiac and lung injury was reproduced by acute cardiac infraction ligating left coronary artery for 4 weeks.Cardiac function were assayed by hemodynamics,heart index and lung index were detected by weighting,pathologic change was checked out by histophathology,and cardiac infarction size was assayed by image analysis system.Results:After operation 4 weeks,cardiac function was disturbed according to the results of hemodynamics.Cardiac and lung index,and cardiac infarction size was significantly increased in model group.Results of histopathology indicated that cardiac myocyte decrease,cardiac fibrosis functional disorder of the heart,increase of heart index and lun index,inflammatory cell infiltration,and pulmonary congestion and edema and so on indicated the model was succeed.Continued administration of n-butanol extract from RP for 4 weeks can remarkable ameliorate cardiac function,decrease the cardiac and lung index,and attenuate the heart failure pathological change.Conclusion:N-butanol extract from RP can alleviate rat cardiopulmonary injury induced by acute myocardial infarction.The present research provided the pharmacological base of replenishing qi for invigorating the spleen and promoting body fluid production for nourishing lung.

objective:To investigate the effect and mechanism of Jianguguben capsule on osteoporosis induced by retinoic acid in rats.Methods:The rats were given retinoic acid 75mg(kg/d)for 2 weeks by intragastric administration,at the same time treated with different drugs according to various groups consecutive for 4 weeks.The bone mine density(BMD)and bone mine capacity(BMC)were assayed by double energy X-ray bone-densitometry equipment,the blood was collected to assay the activity of StrACP in serum,and the hydroxyproline(HOP)contents of femur by commercial kits,the bone’s biomechanics of femur were detected.Results:Jianguguben capsule could significantly improve the bone’s biomechanics,increase the BMD, BMC,index of dry femur’weight,the contents of HOP in femur significantly,and remarkable decrease the level of StrACP in serum of the osteoporotic rats induced by retinoic acid.Conclusion:Jiangguguben capsule has anti-osteoporotic effect on osteoporotic rats induced by retinoic acid.
ABSTRACT
<p><b>OBJECTIVE</b>To analyse sonographical features of epididymis tuberculosis with caseous necrosis and improve the accuracy of ultrasonic diagnosis.</p><p><b>METHODS</b>Ultrasonic features of 12 cases with epididymis tuberculosis with caseous necrosis, confirmed by surgical pathology, was retrospectively analysed: size, location, echogenicity, degree of blood flow in the lesion.</p><p><b>RESULTS</b>In all 12 patients, the number of purified protein derivative test (PPD) presenting the positive reached to 67%, while cases with the pulmonary tuberculosis was 42%. The epididymal tuberculosis with caseous necrosis could present images of the whole or focal epididymal enlargement and abnormal shape. The lesions internal echoes were irregular, inhomogeneous increased isoechoic echogenicity or hypoechoic, including bad-defined, irregular, homogeneous hypoechoic or anechoic. In lesional location, 4 out of 12 patients (33%) possessed richly color blood flow in Color Doppler Power Imaging(CDPI). The testicular enlargement with increased internal echoes and the testicular hydroceles were partly (58%, 7/12) observed by ultrasound. There was no varicocele, scrotal wall thickening and inguinal lymph node enlargement.</p><p><b>CONCLUSION</b>The High Frequency Color Doppler images is helpful for the diagnosis and differential diagnosis of epididymis tuberculosis with caseous necrosis.</p>

AIM: To investigate the effects of radix pseudostellariae on cardiac function and matrix metalloproteinases(MMPs) in rat chronic heart failure induced by coronary artery ligation.METHODS: Rat model of chronic heart failure was produced by ligating the coronary artery for 6 weeks.The cardiac functions were determined by observing the hemodynamics parameters.mRNA expressions of MMP-2 and MMP-9 in left ventricular tissue were detected by RT-PCR and activities of MMPs were assayed by zemography.RESULTS: Cardiac function was remarkable turbulence after coronary artery ligation 6 weeks,according to the results of the hemodynamics.The mRNA contents and activities of MMP-2 and MMP-9 in left ventricular tissue were significantly elevated.Radix pseudostellariae remarkable ameliorated the cardiac functions,inhibited the mRNA contents and activities of MMP-2 and MMP-9 in left ventricular tissue of rat with chronic heart failure after continued 5 weeks treated with decoction of radix pseudostellariae.CONCLUSION: The decoction of radix pseudostellariae ameliorates cardiac functions of rat with chronic heart failure.The mechanism maybe involves in the inhibition of the MMP-2 and MMP-9 mRNA expression and activities in cardiac ventricles.
